Who is covered for DVA Gold Card community nursing.
DVA has several programs, and it's hard to know which one pays for what. If you hold a Gold Card, or a White Card for an accepted condition, DVA pays for the nursing care you're assessed as needing at home. You pay nothing. If any care isn't covered, we'll tell you before it starts.

The colour of your DVA card decides what's covered. Not sure what DVA has accepted on your White Card? We'll check with DVA for you before care starts.

We check your cover with DVA before the first visit
If any care isn't covered, we'll tell you before it starts
No DVA card? DVA doesn't pay for community nursing without one. My Aged Care is the place to start, on 1800 200 422. You're also welcome to call us, and we'll talk it through.
These are two different DVA programs, with two different ways in. Asking for the wrong one can hold up help at home. Nursing care comes through DVA Community Nursing. Help with cleaning, shopping or the garden comes through Veterans' Home Care.

Your first visit
01 / 04
You meet a Registered Nurse
A Registered Nurse comes to your home for the first visit. Family are welcome to be there too. Have your DVA card and any hospital letters handy.
We ask what help you need
The nurse asks about your health, your home and your day. They'll check what you can do easily and where a hand would make things easier.
We agree the plan with you
You and the nurse write the care plan together. It says who is coming, how often and what we'll do. With your consent, we share it with your GP.
Our nurses visit regularly
Our nurses visit as the plan sets out. As your needs change, we update the plan with you. With your consent, we let your GP know when something needs a doctor.
For families
How to get started.
You don't need to fill in any DVA forms. Your GP or hospital team sends the referral, and we do the rest.

Common questions
Questions families and referrers ask.
Here are the questions we hear most. If yours isn't here, give us a call. We're happy to talk it through.
Who can refer to 3 Star Harmony?
A GP, a treating doctor in hospital, a hospital discharge planner or a nurse practitioner. They send the referral straight to us, on DVA form D9389 or their own letterhead. OTs, social workers and families can't refer directly, so start with one of those four.
What does it cost the client?
If DVA covers the care, the client pays nothing. That means a Gold Card with an assessed clinical need, or a White Card for a condition DVA has accepted. We claim from DVA. If any care isn't covered, we'll tell you before it starts.
How will I know if you can take the client?
Every referral gets a reply from us. We'll let you know whether we can take the client and when the first visit will be. If we can't, we'll tell you straight away and explain why, so you can refer elsewhere.
Does the GP stay informed?
Yes. With the client's consent, we let the GP know what we found after the first home assessment. We also update the GP when needs change and when care ends.
How do I get a referral?
Ask your GP. If you're in hospital, ask the ward doctor or the discharge planner. They send the referral straight to us. You can ask for 3 Star Harmony Solutions by name.
Can I choose my provider?
Yes. You can choose which community nursing provider you use. Tell your GP you'd like 3 Star Harmony Solutions, and they'll send the referral to us. If you want to change providers later, talk to your GP.
Who will visit me at home?
A Registered Nurse visits first. They'll get to know you, ask what help you'd like, and write the care plan with you. We'll let you know who's coming and when.
Are war widows and widowers covered?
Yes. War widows and widowers with a Gold Card are covered in the same way as veterans. That means nursing care for any assessed clinical need, at no cost to them.
What if I only need help with cleaning or shopping?
That's Veterans' Home Care (VHC), a separate DVA program. It covers practical help such as cleaning, shopping and home maintenance. You don't need a GP for it. Call 1300 550 450 to arrange a VHC assessment.
Can I use Support at Home and community nursing together?
Sometimes. You may be able to use both, as long as they don't cover the same care. It depends on what help each one gives you. We'll check with DVA before care starts.
What if I don't have a DVA card?
DVA doesn't pay for community nursing without a Gold Card or White Card. My Aged Care can help you find other support at home. Call them on 1800 200 422, or give us a call and we'll talk it through.
