How to send a DVA community nursing referral.

A GP, hospital doctor, discharge planner or nurse practitioner can refer. The referral comes straight to us. You don't send it to DVA. We'll reply and tell you if we can take the client.

Checklist

What to include in your referral form or letter.

Use DVA form D9389 or your own letterhead. It comes straight to us. If anything's missing, we'll get in touch.

  1. 01 Your name, role and provider number
  2. 02 The client's name, DVA file number and card type (Gold Card or White Card)
  3. 03 The condition and clinical details
  4. 04 The nursing care you're asking for
  5. 05 A signed medication chart, if we're to give medicines
  6. 06 The client's level of independence
  7. 07 Other services already in place
  8. 08 Aged care assessment status
  9. 09 The discharge date, if they're leaving hospital

What happens once we have your referral.

We'll update you as each step happens. If we can't take the client, we'll tell you straight away and give you the reason.

  • Who can refer

    A GP, a treating doctor in hospital, a hospital discharge planner or a nurse practitioner. They send the referral straight to us. It doesn't need to go through DVA.

  • OTs and social workers

    You can get a referral started. Talk to the discharge planner or the client's GP. A social worker can refer when they're working as the hospital discharge planner. Share the checklist on this page.

  • Families and clients

    Your GP is the place to start. Ask them about a referral for community nursing at home. Call us if you'd like help with what to ask. We're happy to explain.

  • If we can't take the client

    We'll tell you straight away, by phone and in writing. We'll give you the reason too. That way you can refer elsewhere without losing time.
