These terms and conditions explain how our website and our community nursing service work. They cover referrals, costs, home visits, consent and complaints. Please read them before your care starts. If anything is unclear, call us and we'll talk it through.
By using this website, or by receiving care from us, you agree to these terms. Our Privacy Policy explains how we handle your personal and health information.
Who we are
3 Star Harmony Solutions Pty Ltd (ABN 16 685 808 262) provides community nursing at home for DVA clients. We are based in Lalor VIC 3075. In these terms, "we", "us" and "our" mean 3 Star Harmony Solutions Pty Ltd. "You" means the person receiving care, or anyone acting for them.
This is not an emergency service
We visit at planned times. We can't respond to emergencies. If you or someone else is in danger or needs urgent medical help, call 000 straight away.
Using this website
The information on this website is general. It is not medical advice for your situation. Please speak with your GP or our nursing team about your own health.
We work to keep this website accurate and up to date. Some details may change over time. We may link to other websites, such as DVA. We don't control those sites and are not responsible for their content.
The words, images and logo on this website belong to us. Please don't copy or reuse them without our written permission. Please use the website fairly. Don't try to break it or get into parts you are not allowed to see.
Our community nursing service
We provide DVA-funded community nursing in your home. Care starts after an authorised referral and an assessment by one of our Registered Nurses. Your nursing plan is based on that assessment.
We provide care where it is clinically appropriate. Our nurses use their professional judgement at every visit. We can't promise a particular health outcome.
Referrals and your nursing assessment
Care starts with a referral. It can come from a GP or a nurse practitioner. It can also come from a doctor or discharge planner in hospital. The referral comes straight to us. You can read more on our referral page.
We may not be able to accept every referral. If we can't, we will tell the referrer straight away. We will also let you know and explain why.
What care costs
If DVA covers the care, the client pays nothing. We claim from DVA. A Gold Card covers care for an assessed clinical need. A White Card covers care for conditions DVA has accepted. You can read more on our eligibility page.
If any care isn't covered, we'll tell you before it starts. We'll talk with you and your GP about your options.
Home visits
Our nurses come to your home. To keep everyone safe, we ask you to:
- Keep pets secured during the visit
- Keep paths and entry areas clear and well lit
- Give our nurse a clean, safe place to work
- Avoid smoking in the room during the visit
- Tell us about any risks in the home before we arrive
Our nurses must feel safe to give care. If a visit becomes unsafe for you or our nurse, we may end it. We will contact you afterwards to talk about next steps.
Cancelling or changing a visit
Plans change, and that's fine. If you need to cancel or change a visit, please call us as early as you can. This helps us plan our nurses' days and see other clients.
Sometimes we may need to change a visit time. If that happens, we will contact you as soon as we can to arrange a new time.
Your consent and your right to say no
We ask for your consent before we give care. Our nurses will explain what they plan to do and why. You can ask questions at any time.
You have the right to refuse any part of your care. You can also stop our service at any time. Just tell us. If you stop, we may let your GP know, with your consent, so your care can continue safely.
A family member or carer can act for you if you have given them permission, or if they have legal authority to do so.
Working with your GP and other providers
Good care means working together. With your consent, we share information with your GP and your referrer. We also share it with other providers who care for you. We also report to DVA as needed to claim for your care.
Confidentiality and its limits
We keep your health information private. We handle it as set out in our Privacy Policy.
There are times when the law requires or allows us to share information without your consent. These include:
- When there is a serious risk of harm to you or someone else
- When we must make a report to protect a child
- When a health practitioner must be reported to AHPRA under the Health Practitioner Regulation National Law
- When a court order, subpoena or other law requires it
Where it is safe and lawful, we will tell you before we share information in these cases.
Your rights under Australian Consumer Law
Nothing in these Terms excludes, restricts, or modifies any right or remedy you may have under the Australian Consumer Law.
Limits on our liability
We owe you a professional duty of care. Nothing in these terms limits that duty.
To the extent the law allows, we are not responsible for:
- Problems caused by information that is incomplete or incorrect when it is given to us
- Delays caused by events outside our control
- Indirect or consequential loss
- The content of other websites we link to
Complaints and feedback
We want to hear from you if something isn't right. Please call or email us first. Our details are at the bottom of this page. We will listen, look into it and reply to you.

Changes to these terms
We may update these terms from time to time. The latest version will always be on this page, with the date it was last updated. If a change affects your care, we will tell you.
Governing law
These terms are governed by the laws of Victoria. Any dispute will be dealt with by the courts of Victoria, Australia.
